I think there was a drawing posted at some point about Itachi's original concept design that looked a little like a young Danzo, and people speculated that the design was later re-worked to become part of Danzo.

Danzo Shimura was the leader of Root in the Naruto universe. Considered to be one of the series' most irredeemable villains, he is responsible for much of the conflict that exists in the story. For example, his actions helped to corrupt and influence Kabuto, Nagato, and to a serious extent, even Sasuke.

There's probably a theory out there somewhere that Kotetsu was a clone Danzo created of himself. With all the weird shit going on at the Foundation, and Danzo's dealings with Orochimaru, it honestly wouldn't surprise me. Or get this. It could be something simple like his love child :) That he made with his clone.
